Do I need an ID card?
You do not need an ID card to use your dental or vision benefits. Tell the provider what plan you have, and they will take care of the rest. If you don’t know what plan you have, log in to your online account or call us for assistance.
When does my coverage start?
You coverage begins on the policy effective date. Log in to your account, call us, or talk to your plan administrator to find out when your coverage is effective.
What if my provider is not in-network?
You can still see out-of-network providers, but your costs may be higher. To save money, find an in-network provider online or call us for help.

Log in to your account to view, print or manage your membership roster.
What should I do about an invoice question?
Reach out to your designated representative, and they can help address invoice questions. If you don’t know who your contact is, call our customer care team, and they will connect you.

How do I verify a member’s eligibility or check the status of a claim?
Log in to the dental provider portal to access member eligibility, claim history and claim status. For assistance, call us at (855) 806-5198.

How do I submit a claim?
Submit claims using payer ID “SDCOM” for clearinghouses ChangeHealth, DentalXChange, SmartData and TESIA. Please log in to the Provider Portal to submit claims, or Contact Us for more options.

How do I credential a new associate dentist?
Download and complete the Nevada Credentialing Form. Email the completed form to:
credentialing@primecarebenefits.com or fax to: (702) 852-0260. Be sure to include:
Nevada State Dental License (wallet-size copy with expiration date)
DEA and NV Pharmacy Certificates
Declaration Page of Professional Liability Policy
How do I update my office information?
